Amoxicillin trihydrate
Indications/Uses
Upper resp tract infections eg, ENT infections, otitis media; lower resp tract infections eg, acute exacerbations of chronic bronchitis, lobar & bronchopneumonia; GIT infections eg, typhoid & paratyphoid fever; GUT infections eg, cystitis, urethritis, pyelonephritis, bacteriuria in pregnancy, septic abortion, puerperal sepsis; other infections including Borreliosis (B. burgdorferi) (Lyme disease), skin & soft tissue infections; biliary tract, bone & pelvic infections; gonorrhoea (non-penicillinase producing strains); septicaemia; endocarditis; meningitis; peritonitis; dental abscess (as an adjunct to surgical management); H. pylori eradication in peptic (duodenal & gastric) ulcer disease; prophylaxis of endocarditis.
Dosage/Direction for Use
Adults & childn >40 kg 250 mg tid, increasing to 500 mg tid for more severe infections. Max of 6 g daily in divided doses. Treatment of severe or recurrent purulent infection of the resp tract 3 g bid. Simple acute UTI Two 3 g doses w/ 10-12 hr between the doses. Dental abscess Two 3 g doses w/ 8 hr between the doses. Gonorrhoea Single 3 g dose. Eradication of H. pylori 750 mg-1 g bid in combination w/ a PPI (eg, omeprazole, lansoprazole) & another antibiotic (eg, clarithromycin, metronidazole) for 7 days. Childn <40 kg 125 mg tid, increasing to 250 mg tid for more severe infections. Acute otitis media 750 mg bid for 2 days.
Administration
May be taken with or without food: May be taken w/ meals for better absorption & to reduce GI discomfort.
Contraindications
Hypersensitivity to β-lactam antibiotics (eg, penicillins, cephalosporins).
Special Precautions
Previous hypersensitivity reactions to penicillins or cephalosporins. Discontinue if an allergenic reaction occurs. Avoid if infectious mononucleosis is suspected. Prolonged may result in overgrowth of non-susceptible organisms. Pseudomembranous colitis; discontinue if prolonged or significant diarrhea occurs or experiences abdominal cramps. Renal impairment; reduced urine output crystalluria; maintain adequate fluid intake & urinary output. Monitor when in concomitant w/ oral anticoagulants. Pregnancy.
Adverse Reactions
Drug Interactions
Decreased renal tubular secretion w/ probenecid. Lower oestrogen reabsorption & reduced efficacy of combined OCs. Increased likelihood of allergic skin reactions w/ allopurinol. False +ve readings are common w/ chemical methods. Increased international normalised ratio w/ acenocoumarol or warfarin.
